CHRIST EMBASSY HEALING SCHOOL CLASSES CONSENT FORM AND INDEMNITY AGREEMENT

I, the undersigned, understand that the Healing School Class is absolutely FREE and is open to anyone who seeks divine healing from God in the Name of Jesus Christ. I understand that I have a role to play in my healing by acting on my faith. This is because there are two sides to healing; God's side and, my side. I understand that the subject of healing is a matter of individual faith and belief.

I also understand and agree to the fact that no student is required to stay for more than 2 weeks.

I declare that my presence and attendance at the Christ Embassy Healing School Classes is of my own freewill, and is based on my personal faith and belief in Jesus Christ, His healing power, and His ability to heal me. I also believe in the practical demonstration of my faith based on the Bible, wherein if I had a disability, I would demonstrate my faith in the power of God at work in me, by doing what I could not do before. I, the undersigned, also agree and understand that there are risks in life, and accidents do occur.

I hereby indemnify and save harmless the Christ Embassy Healing School Class and its representatives from any liability, claim, action, harm, injury, loss, fatal event that may occur while I and/or my guardian(s) are on the premises of the Healing School. I further indemnify the Healing School from any liability, claim, action, harm, injury, loss, fatal event that may occur as a result of a voluntary practical demonstration of my faith, which may involve me doing physical things I may not have been able to do in the past as a result of sickness.

I hereby consent to have my picture taken, my interview and testimony recorded on video camera; and for the same interview and testimony to be shown on television and other broadcast media, so that others may know of the reality of miracles and the power in the Name of Jesus Christ.

I hereby consent to the processing of my data in its entirety for the purpose of communication which include follow-up correspondences, notifications, newsletters, magazines, partnership information, promotion of products, programs and services of the Healing School, social media and others namely [the proper administration of the Healing School website and business].

I hereby consent that my data in its entirety be retained by the Healing School, subject to my rights contained in the Terms & Conditions and Privacy Policy @ www.enterthehealingschool.org/policy.php in perpetuity.

I will notify the Healing School of any changes required to the above agreements according to my rights contained in the Terms & Conditions and Privacy Policy @ www.enterthehealingschool.org/policy.php
